REFERRAL PROCESS AND FORM

 STEP 1: To make a referral for Occupational Therapy Support, click the link below to fill out your details to enquire and begin our intake process. This form gathers preliminary information which allows our administration team to begin triaging. 

STEP 2:  Once your referral form has been received, we will contact you within 48 hours to conduct an intake interview in which we will further discuss your referral information as well as discuss an appropriate clinical process for your family.

If Tiny Triumphs has availability following your Intake Interview, you will be booked in for an Initial Appointment. 

You will be sent a Welcome Letter & Service Agreement prior to engaging in your first appointment.
